Participant Waiver, Release of Liability & Parent/Guardian Consent

1. Acknowledgment and assumption of risk

I understand that participation in soccer and related athletic activities involves inherent risks, including but not limited to running, jumping, kicking, falling, collisions with other participants, contact with soccer balls or equipment, and injuries associated with physical activity and use of athletic facilities.

Possible injuries may include cuts, bruises, sprains, strains, fractures, concussions, or other injuries, including serious injury in rare circumstances.

I voluntarily choose to allow my child to participate in Super Soccer activities and knowingly assume the inherent risks associated with participation.

2. Participant health & safety

I certify that my child is physically able to participate in soccer and related athletic activities.

I agree to inform the coach before participation of any relevant medical condition, allergy, injury, physical limitation, or other circumstance that may affect my child's ability to safely participate.

I understand that I am responsible for determining whether my child is medically fit to participate and should consult a qualified medical professional if I have concerns about my child's health or ability to participate.

3. Release and waiver of liability

To the fullest extent permitted by California law, I, on behalf of myself and my child, release and hold harmless Super Soccer, its owner/operator, coaches, instructors, volunteers, representatives, and agents from claims arising from the ordinary negligence of the released parties in connection with my child's participation in Super Soccer activities.

This release is intended to apply to the inherent and reasonably foreseeable risks associated with soccer training and related activities.

This release does not apply to gross negligence, reckless conduct, intentional misconduct, or any liability that cannot legally be waived under California law.

4. Emergency medical authorization

If I cannot be reached in the event of an emergency, I authorize Super Soccer staff or representatives to contact emergency medical services and obtain reasonably necessary emergency medical care for my child.

I understand that Super Soccer does not provide health or medical insurance for participants and that I am responsible for medical expenses incurred on behalf of my child.

Relevant allergies, medical conditions, medications or other information staff should know can be listed in the registration form above. If none, write NONE.

5. Participant conduct

I understand that Super Soccer is committed to providing a positive, safe, and respectful environment.

Participants are expected to follow coach instructions, treat other participants respectfully, use equipment appropriately, and avoid behavior that could create an unreasonable risk of injury to themselves or others.

I understand that a participant may be asked to sit out an activity or, when reasonably necessary for safety, be removed from a session for unsafe or seriously disruptive behavior.

6. Personal property

I understand that Super Soccer is not responsible for lost, stolen, or damaged personal belongings brought to training sessions.

7. Photo & video consent

By registering my child for Super Soccer, I grant Super Soccer permission to photograph and/or record my child during program activities.

I authorize Super Soccer to use these photographs and recordings for legitimate program and promotional purposes, including the Super Soccer website, social media, digital advertising, printed materials, and other program-related communications.

Super Soccer will not identify my child by full name or disclose my child's personal information in connection with these photographs or recordings without additional parent or legal guardian permission.

If I do not wish for my child to be photographed or recorded, I understand that I may opt out by notifying Super Soccer in writing prior to my child's participation. Opting out will not affect my child's ability to participate.

8. Acknowledgment

By signing below, I confirm that: I am the participant's parent or legal guardian; I have read and understand this agreement; I understand the inherent risks associated with participation in soccer and athletic activities; I voluntarily allow my child to participate; I have had the opportunity to ask questions before signing; I understand that this document contains a release and waiver of certain legal rights; and I understand and acknowledge the photo and video consent provisions described above, including my ability to opt out by notifying Super Soccer in writing.
